Names and doses if you have them, including anything prescribed by another clinician and anything you buy over the counter.
What has been happening
Roughly when it started and what has changed. It does not need to be organised. That is what the appointment is for.
Referring a patient?
Clinicians can send a referral through the referral form, or call the office to discuss it first.
Your availability
A few times that would work, and whether you would prefer to be seen in the office or by secure video.
