Clinician referral

For primary care providers, therapists, schools and colleagues referring a patient to Lampstand Behavioral Health. Patients can also refer themselves.

1. Send the referral

Use the form below, or call the office if it is urgent or complicated enough to be worth a conversation first.

2. We contact the patient

The practice reaches out directly to arrange the first appointment, in the office or by secure video.

3. You get an update

With the patient’s written consent, we will confirm that they were seen and share what is relevant to their ongoing care.

Ordinary email is not a secure channel. Send only what is needed to make contact, and send clinical records through a secure route agreed with the practice.

Consent and confidentiality. Please make sure the patient knows you are referring them and agrees to it. Do not send detailed clinical history through this form. Keep it to enough for us to make contact, and arrange secure transfer of records separately.

This is not a crisis pathway. Lampstand Behavioral Health is an outpatient practice and does not provide emergency or after-hours cover. If your patient is at immediate risk, direct them to 911, 988 or the nearest emergency department.
